This is likely fixed by the fix for bug 32843, which doesn't allow uploading the same filename. Though really, this should be prevented by the filename uniqueness check in the "Describe" step. Were you able to get into contact with the uploader?

Well, without any current indication that this happens or any further details pointing to a specific cause, I'll lower the priority for now. Thanks for the help, Saibo.

However, I think that this bug was caused probably because of bug 32843 which wasn't fixed by then. The user must have tried to upload the same file twice and it must have gone through. So I am up for closing this as Invalid?
