Headings created with <hN> tags (e.g.: <h4>Title</h4>) are being listed on Table
of Contents of pages. Check URL for an example, you'll see "# 3.5.3 This header
has the h4 font, but is NOT in the Table of Contents" among its entries.

Don't know if this is a software bug or an outdated documentation.
