Not quite sure it was a plugin, it could have been user error, cut and pasting the wrong thing. The user did correct them self in the next edit.

Yeah, it could be. James had confirmed my theory and that's why I filed it anyway. But I noticed the revert and was really glad about it. Success, right? :)

If they'd pasted the HTML raw it would have refused (<iframe>s etc. are blocked on paste); maybe they tried to paste it as a link target?
