To replace a file it is currently neccessary to delete the file from the page and insert the new one.

It would be easier to replace the filename only. This avoids to add caption and file options again.

I'm not sure there's a great way to merge the experience of adding a media item and the one for editing one, but this seems a reasonable extension.
