If I put in a title and Commons already has a file with the same name and extension, the app will overwrite the existing file with my upload.

Would 'append a timestamp in case of conflict' be a good idea? Or 'prompt user to rename'?

I think prompt user to rename would be better, with instructions to be more specific. That way we're likely to get more useful filenames, rather than a mess of File:Cat-2013-02-27-08:23:32.jpeg files.
