Some server error messages are not correctly sent or processed during the /Continue phase.

On example (debug file on the Server):

further examples: blank page on the consumer wiki when trying to add a non-existing OpenID 

http://exampleprovider.org/wiki/Special:OpenIDIdentifier/id (instead of http://exampleprovider.org/wiki/Special:OpenIDServer/id ) or
http://exampleprovider.org/wiki/Special:OpenIDIdentifier/1234

where "1234" does not exist.
