In contrast to wikitext pages, item pages have no edit functionality when showing an old revision. It would be useful to instead show a (restore) link that can be used to restore the revision displayed. We already have this in diff views, but it's missing from old rev views.
Also include a (restore) link in the page history.
You mean (reset) link to action=reset?
I mean a (restore) link to action=edit&restore=12345 which then gets handled by Wikibase\EditEntityAction. But yea it's close.
Where it should be located? As a button between "Read" and "View history"?
I guess that is a reasonable place - and similar to where it is for normal wiki pages. And it is not a one click action afterwards, but has to be confirmed, so I'd say "Yes, a link between Read and View history" is good.
