As far as request 33377 is "litlle bit" outdated, I decided to open this one - I have just received the consensus page - Hello, please enable the "Article Feedback" extension to Georgian Wikipedia. You might want to see bug 43892 too..
Do you mean https://www.mediawiki.org/wiki/Extension:ArticleFeedback (looks like https://commons.wikimedia.org/wiki/File:Aft-version4.png with stars for rating). It is still deployed (e.g. on some pages on English Wikipedia), but no longer maintained for new features? Or, do you mean https://www.mediawiki.org/wiki/Extension:ArticleFeedbackv5 (looks like https://www.mediawiki.org/wiki/File:AFT5-Feedback-Form-Option-1-Launch-Screenshot.png with no stars, and "Did you find what you were looking for?" ). This is the newer one.

There is a discussion in bug 61163 on whether small wikis should be allowed to use or test ArticleFeedback; your views are welcome.
