Risker reported issues with the Suppression Log for oversighters.

It appears that some oversight-related activities are not formatted correctly and are showing up as confusing messages such as these: 

21:55, 19 June 2012 Thehelpfulone (talk | contribs | block) <suppress-log-request> (test foo bah meh)
19:49, 19 June 2012 Cmcmahon (talk | contribs | block) <suppress-log-unoversight> (bargle)
19:49, 19 June 2012 Cmcmahon (talk | contribs | block) <suppress-log-oversight> (argle) 

What should be shown instead is this:

<date> <user name> (<talk> | <contribs>) un-requested oversight for <'feedback post' # ID> on <article name> <i>(<note>)</i>
<date> <user name> (<talk> | <contribs>) declined oversight for <'feedback post' # ID> on <article name> <i>(<note>)</i>
<date> <user name> (<talk> | <contribs>) oversighted <'feedback post' # ID> on <article name> <i>(<note>)</i>
<date> <user name> (<talk> | <contribs>) un-oversighted <'feedback post' # ID> on <article name> <i>(<note>)</i>
