As of Dec 21 04:03:51 UTC, the file https://en.wikipedia.org/wiki/File:RAF_logo.svg is in an inconsistent state: * Viewing the page shows an image with two uploads in the history. and no image description text. * Viewing https://en.wikipedia.org/w/index.php?title=File:RAF_logo.svg&action=history shows no revision history and states that the page has been deleted. * Viewing http://en.wikipedia.org/w/api.php?action=query&format=xml&prop=imageinfo&titles=File:RAF%20logo.svg&iilimit=10 gives results indicating a deleted image (missing='') with two versions of the image uploaded. * Viewing https://en.wikipedia.org/wiki/Special:Undelete/File:RAF_logo.svg shows a deletion log with two entries from Dec 21 02:57 UTC: one deleting an old revision of the file, and one deleting unspecified content, as well as eight revisions of the image description text. * The file shows up in pages that use it.

What appears is only the image on the page, no page history, no image history, just the most recent image.

Example in comment 0 seems to work correctly now (but I cannot check the deletion log). Does this only refer to files on English Wikipedia, or are there also examples for "bad files" on Commons? Is there any chance As https://en.wikipedia.org/wiki/User_talk:ImageRemovalBot/badfiles was last updated on 28 February 2012 it would be good to know if new cases of this problem are still created or not.
The most recent images I could find that are definitely affected are https://en.wikipedia.org/wiki/File:Picture_470.jpg https://en.wikipedia.org/wiki/File:Zeynep_Ucbasaran_01.jpg https://en.wikipedia.org/wiki/File:1IC8.png from 17 November 2012. ImageRemovalBot identified https://en.wikipedia.org/w/index.php?title=File:Khuda_Aur_Muhabbat.jpg from 6 February 2013 as problematic, but I can't verify it.
