Uploads with bad extensions are warned about but not removed.

If the user attempts to load the same file over again (or another file with an equally unacceptable extension) no subsequent error is triggered.

This also causes strange errors with files that mw.fileApi determines can be readable, but the wiki doesn't want to accept.

fixed in r91656 -- the file input control is cloned and replaced with a fresh one when the contents are bad.
