Despite $wgEmailAuthentication being set to false, account creation results in a
confirmation email being sent.

Subsequent logins work ok without confirmation, as expected.  The URL in the
email cannot be used anyway, as Special:Confirmemail is not a valid page name
when $wgEmailAuthentication is off.
