Rollback has stopped working. Seems a change introduced on midnight's scap (first report 12 minutes after "sync done").
Seems to equally affect all wikis.
Attempts to rollback gives a permission error (token failed).
It isn't fixed by relogging in.

As far as I can tell its caused by r38254.

The token is still correct, but the link no longer includes the &from=LAST_EDITOR

Consistently generates this error for me on en-wp:
Unable to Proceed

There seems to be a problem with your login session;

this action has been canceled as a precaution against session hijacking. Please hit "back" and reload the page you came from, then try again.

Thanks for the info, this allowed me to fix it quickly.  Fixed in r38326.
