It's weekend and #wikimedia-tech is empty as usual, so I'm filing this so that someone can look at it later. Anyway, I reuploaded a new version of [[commons:Image:Wikipedia favicon in Firefox on KDE.png]] at 14:48, 8 November 2008. As of this writing (8 hour later), however, the cached thumbnails for the image still show the old version. I've tried action=purge and clearing my browser cache several times to no avail. On the other hand, the full-size link gives the new version as it should.
Seems to be working now. Per some comments on IRC, it seems to have been a buggy custom thumb handler that's been fixed since. Closing.
