Some images in the Echo UI must be reversed for RTL languages. Of the images currently in the modules/icons directory, these need to be reversed:

Revert
Talk

The ThankYou.png icon in the Thanks extension needs to be reversed, too. Maybe they should be some kind of a generic mechanism for reversing Echo icons.

In theory one could probably use CSS transformations to flip the icons in the browser instead of providing two versions, but it may not be wise to auto-flip.

Amir Ladsgroup proposed a fix in bug 54276 and it seems like a good solution
