This may be one of those weird bugs that only applies to one computer, but... I saw this, a file in use on Wikipedia, but which isn't particularly good. I realised that some level adjustments could at least make it a bit better, so made them and uploaded a new version - with a small, low-res image, I don't think alts are as important. But the original version is all that will show as the newest image. But I can click on previous attempts to upload it, and see my modified image fine, even though, when they were the current version, they showed the original, unaltered form. I even hopped over to a different browser to see if it was something on my end, to no avail. What's going on? http://commons.wikimedia.org/wiki/File:PopeCyril14.jpg
Not entirely clear to me where "the original version is all that will show as the newest image". The picture itself (shown big on top of the page) is definitely your version from 09:33, and also the table has correct sorting here, having your four version on top and sorted correctly (newest first). Firefox 16.0.2 on Fedora 16 here.
How strange: I can't get it to update on my computer.
There's definitely something strange going on with images. I uploaded a file here: https://www.mediawiki.org/wiki/File:Cat_id_rsa.pub.png The size which the HTML generated is forcing my browser to show the image as is correct, but the image shown ( https://upload.wikimedia.org/wikipedia/mediawiki/7/7e/Cat_id_rsa.pub.png ) is the old version. Both the examples of (A mildly mis-titled) bug 41130. In both cases, you can only see the bad if you are not visiting in europe. North America varnish upload caches do not seem to be recieving HTCP purges. In both cases the european squid servers are find. I have no idea why, it really needs ops to look into it. perhaps the varnish servers don't recognize the url as belonging to them. Perhaps the whole htcp -> http purge perl hack thing isn't working properly or can't keep up or something. *** This bug has been marked as a duplicate of bug 41130 ***

In my test of your image, the wrong one (the one that is bigger and has all the mac os ui stuff in it) is only served if visiting upload.wikimedia.org at 208.80.154.235.
upload.wikimedia.org canonical name = upload-lb.eqiad.wikimedia.org. Name: upload-lb.eqiad.wikimedia.org Address: 208.80.154.235 I guess this is because I'm using Google public DNS.
Hmm yep, added "91.198.174.234 upload.wikimedia.org" to /etc/hosts and I'm getting the correct file.
