I have come across what appears to be a broken images upload at http://en.wikipedia.org/wiki/File:LuizEjlli.png which can be viewed (http://upload.wikimedia.org/wikipedia/en/e/ea/LuizEjlli.png) and transcluded on other pages on the English Wikipedia, but doesn't have a file page and cannot be deleted (an attempt to do so brings back an error), moved, or protected (except creation protection). An attempt to create the file page produces a warning notice saying that the image is hosted on Commons, but no such image exists there (http://commons.wikimedia.org/w/index.php?title=File:LuizEjlli.png&action=edit&redlink=1). As it stands, I suspect this image is copyright violation, and it cannot stay visible with no license or copyright information in any case.
Well, I created the page just fine, and filled it in from the upload description. Uploader seems to be active, so feel free to contact them: https://en.wikipedia.org/wiki/Special:Contributions/Rejs12345
The manual creation of the file page seems to have resolved the issue, though that clearly shouldn't have been necessary. I already contacted the uploader when I filed this bug report, however I have now left another message about the image's copyright status.

The image has now been deleted for lack of licensing information.
Bumping up the importance of this bug. Uploads where the file is uploaded but the file description page is not created is a major problem. This is unacceptable (and completely preventable) data loss. Possibly UploadWizard-related? <https://en.wikipedia.org/wiki/File:The_Camden_Head_Front.jpg> is a recent (May 30, 2012) example that was reported here: <https://en.wikipedia.org/w/index.php?title=Wikipedia:Village_pump_(technical)&oldid=495342367#Odd_file>.
In the case I gave when I filled this bug, Brian managed to make the upload page appear by writing some text into the edit window and saving it. I have tried to repeat that for the Camden Head Front example but when I do so the software claims there is an edit conflict, and won't let me save the page whatever I do.
Does this still occur?
