the tooltip for Source can mislead people into thinking that entering "book" is enough. I suggest changing the string to: "Where this digital file came from — Name the URL, book or publication" and it should use an emdash instead of --. I wonder if a smallprint of "This means you release your work under a Creative Commons Attribution ShareAlike license." is actually legally sufficient. Perhaps consult with Creative Commons. And should probably have a link to the CC deed that can open in a new window..... GFDL option doesn't include a version number, unlike the other licenses. Description, there is no margin between the language selector and the textbox. There is no space before the "Learn more" string of its tooltip.
