1. Try to create a new page and click on "Start the <New Page Name> article "
or
1. select an random page
2. Click on edit on the page other than a part

Both occasions it gives an index.php file open blocking both new page create and page edit.

attached the screen print of all the browsers that i have checked this issue. 
Steps followed.
1. Search for a non existing page
2. Click on Create the page <<New page name>> on this wiki!

It works for me. It could be that your browser is caching an empty response from the server. Try searching for and creating other pages, or clearing your browser cache.

Re-closing as WORKSFORME because I can't reproduce it and because it's not related to the UsabilityInitiative extension.

Go to your preferences and disable "Use an external editor"
Your opera screenshot shows clearly that you have that option set.
