MediaWiki usernames currently have their first letter capitalized, regardless how they are entered. This is confusing and undesirable. Many people use lower-case exclusively for such things, and are used to logging on and seeing their user name displayed as they have entered it. Making usernames completely case-insensitive - similar to email addresses - also reduces the chance of spoofing another user. Proper display of full name - if desired - can be accomplished through the Nickname mechanism. I have an entire building full of people with Unix accounts which use lower-case naming almost exclusively. We wanted to set up an internal Wiki using our current logins as accounts, but are unable to do so as MediaWiki subverts whatever we type and capitalizes the first letter. Please change this, or at least provide a LocalSettings variable that can be used to switch this off.
