There are a few languages that have been discussed on Meta's Request for new
languages page, and have a consensus for their creation.

(This list is in format "English name - ISO code - Native name")

Just a detail:
.ml (Malayalam) is not “Malaysian” (the code for Malay language is .ms)

Malayalam is a dravidian language, spoken in Indian state of Kerala, far from
Malaysia (and also by some persons in Singapore, not far from Malaysia) and uses
a specific alphabet (where Malay language is using latin alphabet or Jawi alphabet).

Missing languages have been added to Names.php in r17267.

all old approved new languages have been unapproved following the creation of 
langcom.  please re-do the request there (http://meta.wikimedia.org/wiki/
Requests_for_new_languages) then re-open this bug.

(if they've already been approved, please provide a URL to the langcom 
decision.  kaz has already been created.)
