The history page (&action=history) sometimes doesn't work 
(ends up on "invalid title" message).

But ../wiki/...?action=history does work properly, by 
the way!

Has there been a change to ../w/index.php?

Really odd, like the redirect problem from bug 4923, 
this problem disappeared suddenly! What might have been 
the reason for this? Because it lead to the "Bad title" 
page, it must had been a software or server problem, 
don't you think so?
