If more than one reference has the same content, only one end-note is created.
This can result in a mismatch in number of references and endnotes.

this bug makes it impossable to use the ref system on wiki, it should be
considered a urgent matter as correct citation of sources is highly important to
wikipedia's cred

When I give it the input:

and likewise if I specify a manual key.

Please provide a *simple* testcase (hint: something that isn't ~30KB) and
specify how the output that's being generated differs from what you think it
should generate.

I have not been able to reproduce, including by looking at the the history pages of League of Cambria. Must have been some other strange hiccup.
