The spam blacklist extension will not match URLs which contain underscores.  For  
example, if a line in a blacklist file is:  
  
fiberia\.com  
  
then [http://drugspam.fiberia.com/] will be blocked but  
[http://drug_spam.fiberia.com/] will be ignored.

Added it to the regex.

Reopening since it isn't resolved for non-Wikimedia sites using this extension.
Adding every underscored URL to a separate regex rather than allowing all wiki
admins to add them isn't a solution.

Reclosing since the objection is incorrect. Update your copy of the extension 
and you'll have the updated regex.
