When there are more pages than can show on a page of Special:PrefixIndex the link "Next page (foo)" shows in the upper right hand corner as can be seen here: http://www.mediawiki.org/w/index.php?title=Special%3APrefixIndex&prefix=&namespace=0 However, when no starting page has been entered, the name of the immediately succeeding page is treated as both the &from and the &prefix, frequently resulting in a single entry rather than all of the succeeding pages as expected. This can be seen by clicking the "Next page" link on the page above. When a prefix is given, this does not occur as &prefix is fixed at that value as one pages through the "Next page" links, as can be seen, for example, here: http://meta.wikimedia.org/w/index.php?title=Special:PrefixIndex&from=Centralnotice-2011translations-maryana-1%2Fsr&prefix=Centralnotice-2011&namespace=8 It should be possible to use a null prefix and page through all of the pages in a namespace.
As a related matter, since I can foresee being told that I should be using Special:AllPages (though it's not as easy to use in my opinion), Special:AllPages fails to create a link to "Next page" unless "Display pages ending at" parameter is blank, which means if one uses the navigation tool provided, e.g. http://www.mediawiki.org/wiki/Special:AllPages, one will be taken to a page that has no "next page" link. To get a "next page" link one must either remove the "Display pages ending at" parameter or click the "Previous page" link - after which a "Next page" link can be seen.
