To reproduce, run update.php twice. On the second run, or possibly on the first, you get this:

...cpc_author key already set on code_prop_changes table.
Modifying cpc_attrib field of table code_prop_changes...A database query syntax error has occurred.
The last attempted database query was:
"INSERT INTO `updatelog`( ul_key ) VALUES( 'make cpc_attrib varchar' )
"
from within function "DatabaseBase::sourceFile( /home/catrope/mediawiki/trunk/extensions/CodeReview/archives/codereview-cpc_attrib_varchar.sql )".
Database returned error "1062: Duplicate entry 'make cpc_attrib varchar' for key 'PRIMARY' (localhost)"

Reproduced on trunk, haven't tried REL1_17.
