That looks like a duplicate of #27965 which was fixed in r83814.
But it still occurs with de.wikipedia.org which says to run on  
1.17wmf1 (Version 84096)
(version info is taken from: http://de.wikipedia.org/wiki/Spezial:Version).

To reproduce with de.wikipedia.org:

Step (1) Initial query
http://de.wikipedia.org/w/api.php?action=query&list=categorymembers&cmtitle=Kategorie:Weiblicher_Vorname&cmlimit=10
The query results seem correct, and the cmcontinue (as on March16,21:00) is:
 <categorymembers cmcontinue="5112040" />
Both the list result and the cmcontinue value are as expected.

Step (2) Follow-up query
http://de.wikipedia.org/w/api.php?action=query&list=categorymembers&cmtitle=Kategorie:Weiblicher_Vorname&cmlimit=10&cmcontinue=5112040

That result is mixed up now. The first items are the same as with Step (1). The cmcontinue-item apears in the middle of the list. When testing with different cmlimit values, the behaviour differs (the cmcontinue-item may be even missing in the followup results.)

It is a dupe

But the fixes haven't been put out on the WMF cluster
