A database query syntax error has occurred. This may indicate a bug in the software. The last attempted database query was:
(SQL query hidden)
from within function "CodeRevision::addSignoff". Database returned error "1062: Duplicate entry '3-75547-Reedy-inspected' for key 'cs_repo_rev_user_flag' (localhost)".

2 parts to this maybe..

If a user has already signed off all possible ways, disable button?

And of course, do some checking in code =)

DB error fixed in r77288.
