If you're an administrator on EN, try 
  http://en.wikipedia.org/wiki/Special:Undelete/Image:Math-tetrad-46.GIF 
It fails with the error message 
  Fatal error: Call to a member function on a non-object 
in /usr/local/apache/common-local/php-1.5/includes/Image.php on line 73

Nope, I can't restore [[Image:Math-tetrad-46.GIF]] or [[Commons:Image:Dates.jpeg]].  Have you tried it on EN or the 
Commons?  Perhaps there's something screwed up on those two sites, that isn't broken in CVS.

Ah, restoration does indeed fail as described. Viewing the deleted revision at that URL works fine.
