Steps to produce in en.wikipedia
1) Log out
2) Click Special:UsabilityInitiativeOptIn from "Return to xxxx." in the page.
3) Beta opt-in page does not have "No thank you" button.

This happens when
1) the URL contains no information about where the user came from, so we don't know where to point the "No" button to
2) the user came from Special:Userlogout, and pointing them back there would log them out again. In general, returnto links that would go to the logout page are not shown (also applies to the login success page)

You seem to have hit case 2.
