Ocasionally when the block settings of a username is set to be suppressed from logs and views, the username is not struck off the username list even though it is recorded as sucessful and it is removed from other logs. At other times, the logs are not removed but the username itself is struck off from the username list. When returning to the block settings page again to redo the transaction, the "suppress username" option is shown as unchecked. (a completely successful one is shown as checked)

Whenever you get the "already blocked" page, all checks are reset to the default. Possible related.
