Leaving the beta requires you to fill a form. There should be an option to 'Leave beta without answering'.

 so we would appreciate it if you filled out the optional survey below before clicking "Leave Beta".

Where "Leave Beta" is a link that takes you directly to the submit button on the bottom of the form. There are no required fields in the form, so you can just ignore it. Also note the words "appreciate" and "optional".

Recommend INVALID or WONTFIX since we make it pretty easy for users to skip it already.

The browser, OS and screen resolution fields are already filled for you.
That data can be considered private.

Note that the page doesn't specify what will be done with the provided 
information. Sending the form can be considered a consent to process it.

What is the user expected to do with those fields if he doesn't want to 
provide any information?

If they really, really care, they can clear the resolution fields and set the browser/OS fields to Other. I'd be surprised if anyone actually does care about their browser, OS and resolution being one of tens of thousands of data points in a chart (if they don't fill out any of the actual questions, all we can really use it for is analyze which browsers, OSes and resolutions most people opt out with), though.

You're right that we should have some text in there saying what we use the data for, although it's pretty obvious to me based on the questions.
