This filter http://nl.wikipedia.org/wiki/Speciaal:Misbruikfilter/14 on nl-wikipedia was triggered by this http://nl.wikipedia.org/w/index.php?title=Bestand%3AROA.png&diff=16979271&oldid=2791291 diff. http://nl.wikipedia.org/w/index.php?title=Speciaal:Misbruiklogboek&details=867 reported that there was no old-wikitext, while in fact there was an old description that was edited.
Addition: The user that edited the above diff ([[nl:User:Multichill]]) suggested that the error may have been caused by his working from toolserver.

I've had a poke in the database, seems to be a one-off mistake somewhere along the line. The particular failure mode won't occur again because of internal changes.
