1) Extensions Special Forms  + Semantic MediaWiki works fine. 

2)When adding other extensions as FCKeditor or/and SpecialMultipleUpload, access to Special Pages gives a blank page.

3) Extensions Semantic Media WIki + FCKeditor or/and SpecialMultipleUpload work fine without Special Forms.

4) Changing the order of extensions include or require in localsettings.php file has no effect.

Since you didn't censor this file in any way, you've just made your DB username+password and your $wgSecretKey public. I suggest you change them.

Michel - by "Special Forms" you of course mean "Semantic Forms". If possible, could you change your php.ini file to have "display_errors = on", so that Special:Pages will show an error message instead of a blank page; then write here what that error message is?

You are right, sorry for this mistake

My web hosting doesn't allow me to access php.ini file. I have sent a request to the web host support.

Yaron, sorry, it's only a problem of memory limitation from my web host. Problem is solved. You can close the bug report. Thanks.
