srv78 and srv118 were reinstalled in December according to logs; somehow they seem to be missing the upload5 mount points for the current media file server. As a result they're unable to manipulate file uploads or render math images.

The mount point needs to be set up, and whatever install scripts we've been using should be checked to ensure they are up to date.

I have fixed the two servers.  I will check with mark about fixing the packages.
