During rename process ("Mieciu K" -> "MK wars") account divided into two accounts:
http://pl.wikipedia.org/wiki/Specjalna:Wkład/Mieciu_K (part of edits not moved into new account name) and 
http://pl.wikipedia.org/wiki/Specjalna:Wkład/MK_wars.

Can you merge account or fix in other way?

Users should be logged out when renamed. How was the user still making edits under the old name?

Now it looks ok, all edits have been moved into new username.

IMHO Maybe SUL cause bug - it creat automaticly new account on old username.

Are you saying he accidentally got logged into his global name and started editing under it? I've done that at MW.org a few times...
