A broken skin (named MonoBook2) was left in the skin directory and was user selectable.  Upon a user selecting that skin, any future requests from that particular user account now receive the error:

Fatal error: Cannot redeclare class SkinMonoBook in /var/www/wiki/skins/MonoBook.php on line 33

Utilizing "&useskin=monobook" will allow browsing the wiki while that account is logged in, and the skin section under Special:Preferences can be accessed, but attempting to switch the account to another skin results in the same fatal error upon attempted save.
