I can't remember off hand what the max size is for a picture so I dont know if this exceeds that, but I do remember it throwing a error about that instead of a 500.

This happens when the FastCGI pool on the file server is exhausted, generally due to a chronic overload of the scaling cluster (which may in turn be caused by an overload on the file server itself). It happened yesterday a couple of times when we had scaling servers go down. It was fixed at the time.
