If you use <ref name="xyz">text</ref> in a template, and then you use <ref name="xyz"/> in the body of the article, you get a cite.php  error in the <references/> text. 

To see this, refer to the URL that points to the bad revision in the the article [[Wild Palms]].

This is not a problem in MW.  On the article shown, you were passing an unrecognised field to the template, so this was never output to the page and hence never reached the tag processing stage. Not a bug.
