After having set a new Wiki up from a freshly downloaded 1.12.0 tarball, and adding several Extensions one by one, the wikis main page is shown with MultiUpload added, but Spezial:Version yields:

Fatal error: Class 'UploadForm' not found in .../extensions/MultiUpload/SpecialMultipleUpload.php on line 60

This looks like a bug at first glance.

The wiki is not public atm, yet I can give you access, if you want to.

As you are using 1.12, which version of the extension did you use?

You should be using the 1.12 version from r31252.tar.gz">http://upload.wikimedia.org/ext-dist/MultiUpload-MW1.12-r31252.tar.gz (if this does not work, go to http://www.mediawiki.org/wiki/Special:ExtensionDistributor and only download 1.12 extensions).

Is the issue still there with the proper extension version?

It works for me with MW 1.14 without any changes to the extension.
