In the URL you can see that [[de:User:Ralf Roletschek]] uploaded a new version of that picture. He is an editor, but the new version was not flagged as sighted automatically. As you can also see, automatic flagging does work in general for pictures, so it seems to be just the case of a new upload for an existing file.
