As you can tell by http://en.wikipedia.org/w/index.php?title=Oscar_De_La_Hoya&action=history, user Planecrash111 has undone an article revision credited to me (See http://en.wikipedia.org/wiki/User_talk:Asav). I did not, however, change the article, a user with IP 216.132.15.200 did. This user was responsible for the revision after mine. When you look at the actual revisions one by one, you will see that the changes are registered correctly. In short the software registers the wrong user for some edits.
