I tried to upload a PDF file of exactly 2,568,691 bytes.
Result: Internal error
Message: Could not copy file "" to "/srv/www/wiki/images/a/a0/Xyz.pdf".
I can upload smaller PDF files without any problem.

Check your PHP settings, there's a file size limit for HTTP uploads.

BTW: You should update to 1.3.9 to fix some security issues.
