It seems the reason is that the stored username of that author is empty, so that the link to this author's contributions looks like Special:Contributions?target= which probably renders wrongly on the page history. You can see the empty name on the respective diff page. (I have spotted the error at http://en.wikipedia.org/w/index.php?title=Defenestrations_of_Prague&action=history)
This should only have occurred during a brief time yesterday because of a configuration error. The problem is now fixed (although the blank usernames will still appear in the history). Please reopen if it's still happening.
