I do not want to see people upload files with upper case extension name such as Image.JPG to my MediaWiki installation. Currently, there is no setting option to switch between case-sensitive and case-insensitive.

Would definitely not want this to be a *setting*.

All new uploads should be normalized for sanity's sake; however compatibility should be retained for re-uploading new versions of old files.

I'd like to second that request.

Commons does not allow you to rename an image from $oldname.JPG to $oldname.jpg as $oldname did not change. It should enforce lowercase file extension.
