Currently there are two cases of broken user renaming known on Wikipedia, one on de.wikipedia, and one on en.wikipedia.
In both cases a user was renamed, the old account seems not to exist any longer, but actually there are some contributions left for the old account. 

1) en.wikipedia: 

http://en.wikipedia.org/w/index.php?title=User:RS2007&redirect=no (note the missing contributions link) was renamed to "User:RS1900" on 2007-09-02, see http://en.wikipedia.org/w/index.php?title=Special%3ALog&type=renameuser&user=&page=User%3ARS2007 
But have a look at their contributions:
http://en.wikipedia.org/wiki/Special:Contributions/RS2007
http://en.wikipedia.org/wiki/Special:Contributions/RS1900
There's also a note of the bureaucrat who handled the renaming at http://en.wikipedia.org/wiki/Wikipedia_talk:Changing_username/Archive_2#Broken_rename

2) de.wikipedia:

http://de.wikipedia.org/wiki/Benutzer:Beblawie was renamed to "Weiße Rose" on 2005-11-18, see http://de.wikipedia.org/w/index.php?title=Spezial:Logbuch&type=renameuser&user=MichaelDiederich&page=&pattern=&limit=500&offset=0 (no direct link possible, but you can search for "Beblawie" there)
Now have a look at their contributions:

I'm unsure if this could really be related, but the two cases only have one thing in common. Both renamed accounts have been blocked afterwards, see

So if possible please merge all contributions (including deleted ones) from the 'old' to the 'new' accounts in both cases, and try to find the reason for failure and fix it :-)

Digging up this bug (just in case someone is looking at it), there might be a related issue with my username:

I got renamed from Lucasbfr to Luk on en.wikipedia almost a month ago (04:02, 30 April 2009), but ~5300 edits of mine are still attributed to Lucasbfr:
http://toolserver.org/~interiot/cgi-bin/count_edits?user=Lucasbfr&dbname=enwiki_p

The username was free so I registered it again, but that would be great if you could move all my contribs and log actions to Luk (for obvious GFDL and transparency reasons)

Not quite sure the functionality of the extension UserMerge suits this 
kind of cases or not.

If it suits, it is good to activate that on the WMF wikis.
It is not quite sure is that suitable to be done by bureaucrats on each project 
or needs stewards on meta to be done for it.

Done. Renamed 500 edits (of which 8 deleted) by RS2007 to RS1900.

Done. Renamed 5327 edits (of which 22 deleted) by Lucasbfr to Luk.
