Exactly what functionality would you like to add/override?

Specifically on my Wiki, I need to structure most pages as subpages... therefore the titles come out blah/foo/bar (and I don't like that), and so I am I am planning on restructuring the way titles appear.. 

I don't want to use a skin only hack, because $wgNamespaceswithsubpages setting will create additional output - and if I turn it off, I don't know what implications that might have?
