Basic ConfirmEdit extension (without FancyCaptcha)

Make the captach maths question more prominent.  Maybe a red border or something.

It's too easy (in the default ConfirmEdit set-up) to miss the fact that the edit is being blocked and requires further action. The user might even accidentally close the window, expecting that their edit had saved correctly.

Trivial to fix in my own wiki, but should be improved in the default.
