The separate sections are still listed, but their names do not appear if their
heading is an internal link.  The table of contents ends up looking like this:

Bug fix, look like some change in 1.4 add the need to replace also IWLINK in headings

wgInterwikiLinkHolders must be used to replace == [[w:link]] == interwiki links
in headings, patch apply to 1.4 cvs, bugs not present in 1.3, perhaps needed
for 1.5 too. Tested on 1.4 cvs with my local wiki

I'm not familiar with the way the parser works, I think it's the right way.

Fixed in CVS HEAD and REL1_4
Deployed to wikipedia.org
