Wikimedia Bugzilla is closed!

Wikimedia has migrated from Bugzilla to Phabricator. Bug reports should be created and updated in Wikimedia Phabricator instead. Please create an account in Phabricator and add your Bugzilla email address to it.
Wikimedia Bugzilla is read-only. If you try to edit or create any bug report in Bugzilla you will be shown an intentional error message.
In order to access the Phabricator task corresponding to a Bugzilla report, just remove "static-" from its URL.
You could still run searches in Bugzilla or access your list of votes but bug reports will obviously not be up-to-date in Bugzilla.

I'd like to suggest an extension to MW whereby an inputbox could be placed on a page where the user can enter a page title and click a button to create a page.

Once the title is entered, a "forms" page opens, with drop boxes/input boxes/etc., that direct the user to enter specific information to create the new page. In the case of our wiki (wikiscuba), that could be information like

type of dive site: 
gps coordinates:


On confirming the information entered, the extension incorporates the information given and a template to create the new page.

I'm pretty sure several such extensions exist.

Could you point to such extensions? I spent several hours looking and couldn't find one. The only extensions I found were the ones where a new page would be created with a specified name, but none that would take in more info and create a new page containing that info.
