I'd like to suggest an extension to MW whereby an inputbox could be placed on a page where the user can enter a page title and click a button to create a page. Once the title is entered, a "forms" page opens, with drop boxes/input boxes/etc., that direct the user to enter specific information to create the new page. In the case of our wiki (wikiscuba), that could be information like type of dive site: gps coordinates: depth: etc. On confirming the information entered, the extension incorporates the information given and a template to create the new page.
I'm pretty sure several such extensions exist.

The only extensions I found were the ones where a new page would be created with a specified name, but none that would take in more info and create a new page containing that info.
